Court Requests

Should a clinician from Affinity Counseling Services be legally required, requested, or subpoenaed to appear in court, attend a deposition, or participate in any legal proceeding on your behalf, the following fees apply:

  • Court Appearance Fee: $150.00 per hour.

  • Minimum Charge: A minimum of 4 hours ($600.00) will be charged for any scheduled court date, regardless of the actual time spent testifying or if the case is settled or dismissed on that day.

  • Additional Time: Any time spent beyond the initial 4 hours (including travel time, waiting time, and preparation time) will be billed at the standard $150.00 per hour rate.

  • Payment Terms: The minimum fee of $600.00 must be paid in full at least 72 hours prior to the scheduled court date. This fee is non-refundable if the appearance is canceled without sufficient notice.

Request for Records

To protect the privacy, confidentiality, and integrity of the therapeutic relationship, Affinity Counseling Services maintains a strict policy regarding the release of client documentation for legal proceedings.

  • Subpoena Requirement: Any clinical records, progress notes, or documentation needed for court purposes require a valid subpoena or a specific court order signed by a judge.

  • Processing Fees: Standard administrative fees may apply for compiling, printing, and securely transferring records as permitted by state law.
